Clinical Rationale

A Chopart injury initially managed in plaster collapsed both columns of the foot over eight months. Reconstruction required opening the medial and lateral sides simultaneously and restoring lost lateral column length with a structural iliac crest graft.

Surgical Approach

Reconstruction restored the medial and lateral column lengths through a naviculo-cuneiform fusion combined with a distraction calcaneocuboid fusion, using a structural iliac crest graft to re-establish lateral column length.

A 46-year-old man sustained a major midfoot injury in a road-traffic accident, producing severe swelling and an inability to bear weight. The injury was initially treated in a below-knee plaster, with follow-up radiographs taken through the cast. Eight months later, he presented with persistent pain, swelling and substantial difficulty using the affected foot.
