Clinical Rationale

Pilon, talus and calcaneus fractures on both sides, alongside head and abdominal injuries, raised a real question of whether the patient would walk again. Both hindfeet were fused with a custom straight nail that compresses the ankle and subtalar joints simultaneously.

Surgical Approach

Both sides were reconstructed with tibiotalocalcaneal (TTC) nailing; the left also received primary bone grafting. The centre's own straight nail design allowed nail-assisted compression across both the ankle and subtalar joints, achieving sound union despite severe comminution.

A 37-year-old man sustained devastating injuries to both ankles after falling at a construction site. The trauma involved the pilon, talus and calcaneus on both sides, together with head and blunt abdominal injuries.
