Clinical Rationale
A young working-age patient arrived with both heels shattered in the most severe Sanders IV pattern. Rather than stage the surgery over months and leave him disabled in between, both feet were reconstructed in three dimensions and fused definitively in a single planned sitting.

Surgical Approach
Both feet were treated with the centre's preferred single-stage definitive protocol — three-dimensional reconstruction of the calcaneus followed by primary fusion and structural bone grafting.

Comminuted "bag of bones" calcaneus fractures are among the most difficult to manage; the trend favours reconstruction followed by primary fusion when indicated.

Young, working-age patients suffer prolonged disability in the period before fusion — making single-stage definitive surgery the preferred path.

At the time of fusion, three-dimensional reconstruction of calcaneal shape and size is mandatory, with excision of all small articular fragments.

Bilateral Sanders IV Calcaneus Fracture Reconstruction
A single-stage primary fusion approach to one of the most challenging fracture patterns in foot & ankle surgery — performed on both feet of a young working-age patient.
A 24-year-old adult sustained highly comminuted fractures of both heel bones after falling from the third floor. The patient presented 24 days after the injury, when the fractures were already difficult to address through conventional reconstruction alone. Because both heels were involved, the priority was to create two stable, correctly shaped feet capable of supporting future weight-bearing.
